If the form does not open with clicking the link, please right click and choose "Save Target As" to save a copy to your computer. Open a new document in Microsoft Word, then go to the Insert menu and select Watermark. When the Watermark dialog box appears, click the Picture checkbox and then click on the Select Picture button. Select the PDF you want to fill out and click Insert. Make sure you uncheck the Washout box and leave Scale set to Auto. Click OK, and the PDF will appear as the background of your blank document. Now you can complete and save the form for later use.
